Consent to use of information and material obtained through the WITH Project digital portal

• I understand that the material and information I submit in response to this questionnaire is not intended to be confidential and will be made available for public use on the WITH Project digital portal.
  
• I understand that if I do not wish the material and information I contibute to be made available to the public I can indicate this by NOT selecting the "Publish my contribution on the web" box.
• I agree to clearly indicate to the researcher any sections of the material, matters or information I wish to keep confidential and understand that requests for confidentiality will be respected.
• I understand that the content of what I say in my contribution and any other information or material I provide, is not intended to be anonymous.
• I agree to clearly indicate to the researcher if I want my anonymity to be maintained and understand that requests for anonymity will be respected.
• I understand that I have the right to withdraw any information or material I provide at any time during the course of the study (before December 31, 2021, or at any date agreed on directly with Chief Investigator, Jane Hunt).
• I am aware that I may request feedback on the research.
• I am aware that I can request restrictions on use of the material and information I contribute and have outlined the restrictions I would like to request in the space provided on the contribution page.
